Amtrak OIG-led investigation leads to fraud charges against Chicago chiropractor

CHICAGO – A Chicago-based chiropractor was indicted on federal fraud charges June 3, 2019, for allegedly submitting reimbursement claims to private insurers for nonexistent treatment.

According to court documents, John Kosloski, 55, of Beecher, allegedly submitted fraudulent medical claims to insurance providers for services that were not rendered. Amtrak insurance providers were among those that received the allegedly fraudulent claims.

It was also alleged that Kosloski used personal identifying information of at least five people and their family members to generate fraudulent medical claims. In exchange for using their information, Kosloski allegedly paid the co-schemers via envelopes of cash, according to the indictment.

Kosloski was charged with six counts of health care fraud and pleaded not guilty at his June 3 arraignment before U.S. District Judge Rebecca R. Pallmeyer. Each count of health care fraud is punishable by up to ten years in prison.

Amtrak OIG, the U.S. Attorney’s Office, Northern District of Illinois, and the FBI supported the joint investigation.
