The summary should be brief, but accurate statement of the specific facts you believe indicate wrongdoing. Be as specific as possible as possible about the location of the incident, and the time(s) the activity occurred. The OIG investigates fraud, waste, abuse, and mismanagement within or affecting Amtrak’s operations, whether committed by employees, contractors, and/or third parties. We want to know about any person or entity that steals from Amtrak or its contractors or the Government; fails to protect company assets; provides Amtrak with inferior or substandard products or services; engages in kickbacks, bribes, gratuities, procurement fraud, cheating, and/or double billing; violation of law, rules or regulations, and systems or processes which are being mismanaged or abused. A few examples of what the OIG investigates include, but are not limited to: embezzlement, bid rigging, theft of Amtrak property, bribery, conducting an outside business using company material on company time, retaliation for OIG cooperation, health care fraud, conflicts of interest, etc.
