Waste Mismanagement

The Office of Inspector General ("OIG"), Office of Investigations ("OI"), received information that [REDACTED] Manager OBS, allegedly requested that the Crew Management Services ("CMS") assign [REDACTED] Service Attendant ("SA"), to work an SA job on train no. 30 that runs from Chicago ("CHI") to Washington, D.C. ("WAS"), without properly submitting a Service Request.

LSA Theft-April 24, 2009

During Lead Service Attendant ("LSA") packet reviews by the Office of Inspector General ("OIG"), Office of Investigations ("OI"), it was determined that packets belonging to [REDACTED], former LSA, had contained under reported sales amounts. The packets had been processed by the Train Provision Management System ("TPMS") Clerks who had previously been implicated in activities designed to retain Amtrak funds for personal gain.

Counterfeit Amtrak Coupons

The Office of Inspector General (OIG), Office of Investigations (OI) received an allegation concerning the counterfeiting of Amtrak Guest Reward (AGR) coupons. The complaint allages that the counterfeit AGR coupons were being sold on eBay and then were redeemed for Amtrak travel.

Theft-March 25, 2009

On August 27, 2008, the RA and Senior Special Agent [REDACTED] received from [REDACTED] Union Station, Washington, D.C. an open box she recovered from the baggage area on the lower level of the station. Contained in the box, which had been sitting on a wooden skid in the baggage area, were numerous books of conductor tickets.

Mismanagement

On June 11, 2008, the OIG-OI conducted an investigation to determine if the Amtrak station in Albany, NY, had been out of compliance with Amtrak's Station Service Accounting procedures. As a result of several interviews with Amtrak employees, it determined that there were deficiencies which contributed to the loss of revenue and unauthorized number of duplexes issued to Conductors.

Theft-February 25, 2009

The Office of Inspector General (OIG), Office of Investigations (OI) received an allegation from Amtrak Radio Maintenance [REDACTED] concerning the unauthorized use of Amtrak's Motorola sales account number [REDACTED]. [REDACTED] reported that Amtrak Train Engineer [REDACTED] used the Amtrak account to obtain radio software for an Astro 25 radio, which is not an Amtrak assigned radio.

Mismanagement

Management responded in a timely fashion to OI's referral of March 3, 2009, and acted affirmatively on all the recommendations referenced in said referral. According to said response, a total of $4900 will be collected from the owners of Private Cars [REDACTED] and [REDACTED]. Additionally, management audited all Private Car accounts, and as a result of said audit corrected billing worksheets were completed and sent to the Amtrak Finance Department in Chicago for colletion.
